Students are required to use the Pre-participation – Physical Examination Form provided. NO OTHER PHYSICAL EXAM FORM WILL BE ACCEPTED. A new physical is required for all athletes entering the 7th, 9th, and 11th grades. Athletes entering the 8th, 10th, and 12th grades do not need a new physical UNLESS they did not have one last year or if they have seen a doctor for an injury during the year. The Medical History form MUST be filled out and signed yearly. Physicals dated prior to May 1st may not be accepted.
The required annual athletic physical and health history cannot be filled out online because it must be completed by the parent/guardian and physician. You may print a physical and medical history by selecting the physical and medical history tab at the top of the page. Once you have selected the tab you will see the link to the physical and medical history and will be able to download and print. You are encouraged to fill out the medical history prior to printing the form.
All other required paperwork will be done electronically. The parent/guardian will be asked to provide information such home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically. You must have your son/daughter’s Student ID number in order to fill out the required forms.
Remember that to submit online appropriately, you and your child must and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.
General Electronic Form Instructions:
1. In order to submit online paperwork appropriately, you will be asked to provide information such as home phone number, cell phone numbers, email address, name of primary physician, and other pertinent information. It is very important you have this information available during the time you plan on completing these forms.
2. Remember: It is important that you and your child fill out and sign all required spaces. Forms will not be accepted electronically if the forms are not completed correctly.
3. No student will be able to participate in an athletic period, practice, or games if these forms are not completed and processed. This is a University Interscholastic League rule.
4. To begin, click on the form name and fill out the information requested. It is important you have your student ID number available as it is required on each form. If you do not know the answer to a question, please answer the question with N/A or none.
5. To sign a document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using a touch screen device, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.
6. Once you have filled out all of the information on each page you will have the opportunity to print the document.
7. You will receive a confirmation email once the documents(s) have been reviewed by the El Campo ISD athletic trainer.
